Programme Outline
Serious illness or unexpected changes in health can sometimes leave families facing difficult healthcare decisions on behalf of a loved one.
Advance Care Planning (ACP) provides an opportunity to have these important conversations early, while we are well and able to communicate our preferences.
This session will introduce Advance Care Planning (ACP) and explore how ACP can help individuals reflect on what matters most to them, communicate their healthcare preferences, and prepare their loved ones to support and speak on their behalf should they become unable to make decisions for themselves. Participants will gain a clearer understanding of what Advance Care Planning involves, its role in future healthcare decision-making, and how it differs from a Lasting Power of Attorney (LPA). The session will also address common misconceptions and provide practical guidance on how to begin meaningful conversations with loved ones about personal values, healthcare wishes, and future care preferences.
Through an interactive and reflective session, participants will be encouraged to consider what matters most to them, start meaningful conversations with their loved ones, and take practical steps towards planning for their future healthcare needs.
