Programme Outline
A person in the last few years of life is likely to have to navigate multiple services and care transitions. The refreshed National Strategy for Palliative Care in 2023 have listed 3 focus areas of improving access, enhancing quality and building a supportive palliative care environment to ensure that all Singaporeans, including their caregivers, receive the care they require, in timely, person-centred and seamless ways. This entails that services often collaborate and sometimes integrate to “do the dance around the person”. What does integrated care look like in palliative care? How do we do this dance with multiple dance partners? How do we build trust and collaborations and break down barriers? Join us as we examine the principles of integrated care in palliative care and lessons from real life examples for choreography towards perfect harmony.
