Course Description: Chronic diseases are long-term medical conditions that are generally progressive. Statistics show that Singapore’s ageing population and adoption of unhealthy lifestyles have contributed to the increasing prevalence of chronic diseases. This course is designed for healthcare professional caring for patients with chronic diseases in the community setting. Eligible for 12 CPE points upon successful completion.
