Course Description: This course equips healthcare professionals with the fundamental knowledge and skills needed to manage patients with chronic diseases in primary care settings.Topcis include Overview of Chronic Disease Management, Diabetes, Hypertension , Lipid Disorders, Asthma & Chronic Obstructive Pulmonary Disease (COPD), Dietary Management of Chronic Diseases, Role of Inter-Professional Collaboration (IPC), Introduction to Frailty Management & Modified Bartel Index (MBI) & Screening for common mental health condition.
